Job Description
The Faircape Group owns and manages six luxury retirement / lifestyle villages with state of the art and technologically advanced Healthcare Centres at each Village.
These busy facilities with a versatile range of patients inclusive of frail, mental health, sub acute and assisted living, are situated in prime locations across the Western Cape: (Bridgewater and Heritage Manor -Somerset West, Tokai Estate - Tokai, Cle du Cap Kirstenhof, Noordhoek Manor- Noordhoek and Onrus Manor - Onrusriver.)
We are seeking a Social Worker to provide social work support and services to residents within the healthcares and villages as required. The Social Worker will focus on all six facilities initially to assess and formulate mental health care plans for the residents. Thereafter the successful incumbent will focus on the facilities in the southern suburbs (Cle Du Cap, Tokai Estate and Noordhoek Manor)

Duties will include the following, but not limited to:
- Counselling to healthcare residents
- Support/counselling to residents and/or family members
- Assistance with adjustment to admission or change in health status
- Investigation into allegations of abuse or neglect of any nature
- Assistance with care decision making in absence of family
- Assistance with managing difficult family situations
- Participating in monthly MDT meetings
- Assisting with curatorship applications
- Providing training to various members of staff based on need and request
- Writing health educational/promotional material for newsletters to families and residents
Requirements
Qualification:
- Bachelors Degree in Social Work
- Qualified or experienced in Clinical Social Work
Experience amp; Knowledge:
- 2 years experience in a similar role
- Knowledge pertaining to external private and government frail cares and home care services will be beneficial
- Knowledge of the Older Person Act
- Previous experience with curatorship applications would be beneficial

- Excellent communication skills (verbal and written)

- Ability to establish and maintain collaborative and effective working relationships
- Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills
Specific Requirements:
- Registered with SACSSP
- No criminal record
- Drivers license and own transport
Working Hours: Mondays to Fridays from 08h00 until 17h00
